Nashville, TN — Tennessee Correction Commissioner Frank Strada is facing mounting scrutiny after Christa Pike survived a failed lethal injection, marking the fifth execution described as botched or seriously problematic under his leadership in Tennessee and Arizona.

Pike, 50, remained unconscious and on a ventilator Friday after receiving two doses of pentobarbital. Her attorneys say improperly placed IV lines allowed the drug to enter surrounding tissue rather than her bloodstream, leaving her arms swollen, burned and blistered. Gov. Bill Lee has suspended executions for the remainder of 2026 and ordered an independent review.

Strada previously oversaw execution operations in Arizona, where three executions in 2022 were classified as botched by the Death Penalty Information Center after teams repeatedly struggled to establish IV access. Tennessee then hired him as correction commissioner in 2023.

Since then, Tennessee has experienced two failed execution attempts. Tony Carruthers’ May execution was abandoned after officials spent more than an hour trying to establish required IV access. Pike survived Wednesday’s attempt after lethal drugs were administered.

Critics also note that Tennessee eliminated a previous training requirement involving practice IV insertion under Strada. A 2022 state-commissioned review had already documented serious failures in Tennessee’s execution procedures.
